If you are requesting medical records from the North Broward Medical Center, please note that you will need to complete the Authorization for Release of Confidential Medical Information. Once you have signed and completed this form you can either:
1) Drop it off personally at the Medical Records Department located on the first floor at North Broward Medical Center
2) Mail the form to:

Please note it takes approximately 3 business days from the date we receive your request for completion. If the records are stored off site additional processing time may be required.
