The Enlightened Women's Club is a program started on behalf of the CSMC's Women's Diagnostic and Wellness Center. Here at the Center, we are dedicated to your health and well-being and are pleased to offer you the following membership benefits:

How can I join?
As our gift to you, your lifetime membership in the Enlightened Women's Club (valued at $10) is complimentary. To apply, fill out the online form. You will receive your Enlightened Women's Club package in the mail within four to six weeks. Offer expires May 9, 2010.
How can I become a business partner?
Our business partners are what makes our program so successful. If you would like to show your support as a partner, just fill out the Business Partner Application or call us at (954) 344-3005 if you have any further questions.
