Primary Care

NOTE: For new for first-time Star Card participants, appointments can be processed by contacting our Community Health Services Centralized Scheduling Center at 954-712-3977 between the hours of 8:30 am - 4:30 pm Monday through Friday. Established Star Card participants should contact their assigned health clinic.

Outpatient Diagnostic Procedure

Centralized Scheduling is for appointments for Medical Imaging (i.e. Radiology, Mammography, Computerized Tomography, MRI, Ultrasound, Nuclear Medicine and Pet Scans), Cardio-Pulmonary, Sleep Disorder Studies, and Neurodiagnostic Studies. If you would prefer to contact the Centralized Scheduling Department rather than utilize the electronic request, please call (954) 759-7500.

Some situations require a referral from your doctor. If your insurance requires that you need a referral from your doctor to see an orthopedic physician, STOP HERE and call your primary care doctor directly.
