Website Substitute Notice to Patients

We are providing this notice in accordance with the substitute breach notification provisions of the Florida Information Protection Act concerning a potential breach of certain patient information.

Patient Notice: Data Breach

On May 19, 2017, Broward Health discovered than an employee of a Broward Health vendor sent an unencrypted e-mail to an e-mail address outside of Broward Health. The e-mail contained Protected Health Information (PHI), including patient names, medical record numbers and treatment information. In some instances, the medical record number may have been a Social Security Number.

Since it is possible that we have outdated contact information for six (6) of our former patients, we are giving notice of the incident on our website in accordance with the Florida Information Protection Act (F.S. §501.171). Patients who visited Broward Health North and Broward Health Imperial Point between April 2013 and March 2017 may be affected by this incident. Broward Health has no indication patients seen at other Broward Health facilities are impacted. All other patients affected have been notified via first class mail.

Broward Health takes the protection, privacy, and security of our patients very seriously. All workforce members receive education on the policies and procedures that Broward Health has implemented to safeguard the privacy of our patients. Our Corporate Compliance program monitors activities to assist in ensuring that our workforce members meet the expectations of Broward Health and to provide additional education when needed.

We have taken the following measures to protect against these types of issues from reoccurring in the future:
Terminated our contract with the Broward Health vendor.
We are reviewing and enhancing our information security safeguards.

We are keenly aware of how important your personal information is to you. While Broward Health has no information or reason to believe that your information was misused in any way; you are advised to take the following precautions to protect yourself against any potential misuse of your information:
Monitor any healthcare services Explanation of Benefit forms that you receive to be certain they list services that you received.
Monitor your credit card statements, reports and other banking information for appropriateness of charges and usage.
Contact the three nationwide credit reporting agencies and request a copy of your credit report.
