Cora E. Braynon Family Health Center's Care Coordination Team hosted a mini health fair to build additional awareness concerning diabetes and the onset of diabetes which can lead to cardiovascular disease and other disease manifestations. The Care Coordination Team leveraged their working relationship with the CEBFHC Pharmacy Department to provide nutritional counseling, medication management, blood glucose and pressure screenings to attendees in a community health center setting. CEBFHC was positioned as an access point for primary care among residents living in close proximity to the health center.
