CHS Outreach Services positioned Broward Health and its ancillary service lines as access points for behavioral care targeting local Medicare recipients gathered in a faith based setting who require healthcare, behavioral care and social services. MODCO Senior Connection is committed to providing health education and empowerment strategies to seniors living in central Broward County. Many of these seniors need linkage to Broward Health's ancillary service network. Cora E Braynon Family Health Center was positioned as an access point for delivery of mental health services among this target population.
