CHS Outreach Services positioned Broward Health and its ancillary service lines as access points for primary care targeting local Medicare recipients gathered in a faith based setting, who require healthcare and social services. MODCO Senior Connection is committed to providing health education and empowerment strategies to seniors living in central Broward County. Many of these seniors need linkage to Broward Health's primary and tertiary care service network. Cora E Braynon Family Health Center was positioned as an access point for delivery of primary care services among this target population.
