CHS Outreach Services coordinated and facilitated a follow-up presentation on the benefits of knowing your readings, after screenings, to promote a healthy lifestyle using advice and guidance from BHIP's clinical team. The BHIP's Clinical Team was on hand to inform seniors about other methods of how to maintain their health and wellness using light screenings. Keeping up with key health indicators concerning their healthcare status, by regularly attending physician appointments with their primary care provider (PCP), is of vital importance in maintain health and wellness.
