Matrix Absence Management

Matrix Absence Management administers Family and Medical Leave (FMLA), general medical leave and Military Leave benefits to ensure full compliance with federal regulations.

How to Request Leave

If you expect to be absent due to surgery, a medical procedure or for three or more days for family or personal medical leave, you must contact Matrix Absence Management as soon as possible.

You can initiate the leave process 24 / 7 / 365 through:

For unplanned leave, contact Matrix immediately. For planned leave, contact them up to 30 days in advance.

Leave Verification & Return to Work

  • Matrix Absence Management will work with you and/or your doctor to verify eligibility and manage your leave.
  • Before returning to work, you must bring your completed Fitness-for-Duty/Return to Work Release Form to Broward Health’s Employee Health Department for review, in accordance with the Clearance for Duty/Return to Work Policy. Please note the form was included in your Claim Packet from Matrix and is available for download through your online account with Matrix in the Documents section of your claim.

Personal Leave & Sick Pay

  • Personal Leave and Sick Pay are benefits offered by Broward Health.
  • Sick Pay approval is managed by the Leave Administration Center.
  • If you return to work with restrictions, please email the Leave Admin Center at [email protected] to obtain the accommodation form if not previously provided to you. Please note the form is due within 15 days of receipt and should be submitted to the Leave Admin Center for processing.
  • If you exhaust both your PL and Sick Pay banks, your coworkers can use the PL Donation Form to donate hours to you. The form is located here: Employee Authorization for PL Donation Once complete, the forms should be submitted to your Human Resources office at your region.
  • If you exhaust both your PL and Sick Pay banks, you will be contacted by Chard Snyder to arrange payment for your benefits.

Leadership Communication from Matrix Absence Management

Matrix Absence Management communicates with leadership regarding employees on leave or those who have submitted leave requests via the status notifications report.
Below are the key details leaders should be aware of:

  • Welcome Email Notification

  • Leaders will receive a Welcome Email from [email protected]. This email will inform leaders that an account has been created for them and will include instructions for setting up their password and accessing the Matrix Absence e-Services portal.

  • Accessing the Matrix Absence e-Services Account

  • Leaders can log into their Matrix Absence e-Services Account using their username and password.

  • Additional Security Measures

  • To ensure security, Matrix Absence Management will send a one-time verification code to the leader's Broward Health email address before granting access to the e-Services portal.

  • Information Provided to Leaders

  • In addition to having an account set up, Matrix Absence Management will provide leaders with information about employee leaves in two ways:

    1. Status Notifications

    These emails inform leaders of new or updated information about absences within their team. To ensure that these important emails are received and do not go to the junk mailbox, please add [email protected] to your address book.

    2. Absence Calendar

    The absence calendar offers a detailed, interactive view of employee absences over a 12-month period, allowing leaders to review past absences and plan for future ones. This calendar is customized specifically for each leader, displaying data relevant to their team.
