Website Substitute Notice to Patients

We are providing this notice in accordance with the substitute breach notification provisions of the Florida Information Protection Act concerning a potential breach of certain patient information.

Patient Notice: Data Breach

On August 25, 2017, Broward Health discovered that a former employee of a Broward Health sent an unencrypted e-mail to an e-mail address outside of Broward Health. The e-mail contained personally identifiable information for employees whose employment ended in 2015. The personally identifiable information included the names of the former employees, their employee identification numbers while employed at Broward Health, and a Social Security Number.

Since it is possible that we have outdated contact information for sixty-seven (67) of our former employees, we are giving notice of the incident on our website in accordance with the Florida Information Protection Act (F.S. §501.171). Former employees of Broward Health, whose employment ended in 2015, may be affected by this incident. All other former employees affected have been notified via first class mail.

Broward Health takes the protection, privacy, and security of our patients and employees very seriously. All workforce members receive education on the policies and procedures that Broward Health has implemented to safeguard the privacy of our patients. Our Corporate Compliance program monitors activities to assist in ensuring that our workforce members meet the expectations of Broward Health and to provide additional education when needed.

We have taken the following measures to protect against these types of issues from reoccurring in the future:
The employee responsible for the incident is no longer employed with Broward Health.
We are reviewing and enhancing our information security safeguards.

We are keenly aware of how important your personal information is to you. While Broward Health has no information or reason to believe that your information was misused in any way; you are advised to take the following precautions to protect yourself against any potential misuse of your information:
Monitor any healthcare services Explanation of Benefit forms that you receive to be certain they list services that you received.
Monitor your credit card statements, reports and other banking information for appropriateness of charges and usage.
Contact the three nationwide credit reporting agencies and request a copy of your credit report.
