Voya Leave Management

Voya Leave Management administers Family and Medical Leave (FMLA), general medical leave, and Military Leave benefits to help ensure compliance with federal regulations.

How to Request Leave

If you expect to be absent due to surgery, a medical procedure, or for 24 consecutive hours or more for family or personal medical leave, or if you are requesting a workplace accommodation, you must contact Voya Leave Management as soon as possible.

You can initiate the leave process through:

Online access is available 24/7. Phone support is available Monday–Friday, 7 AM to 9 PM EST.

For unplanned leave, contact Voya immediately. For planned leave, notify your department leader at least 30 days in advance and contact Voya to initiate your leave.

Leave Verification & Return to Work

  • Voya Leave Management will work with you and/or your health care provider to verify eligibility and manage your leave.
  • Before returning to work, you must bring your completed Fitness-for-Duty/Return to Work Release Form to Broward Health’s Employee Health Department for review, in accordance with the Clearance for Duty/Return to Work Policy. The form is provided during the Voya claim process. If you are returning to work with restrictions, please review the accommodation information outlined in the third bullet of the Personal Leave & Sick Pay section below.
  • If you were out on leave for the care of a family member, military leave or the adoption of a child, please contact the Leave Administration Center at 954-473-7701, Monday through Friday from 8:30 AM to 5 PM, to begin the process of restoring your access to Broward Health systems. Failure to follow this process may delay your return to work.

Personal Leave & Sick Pay

  • Personal Leave and Sick Pay are benefits offered by Broward Health.
  • Sick Pay approval is managed by the Leave Administration Center. If Employee Health does not issue a clearance note, your access to Broward Health systems and your return to work may be delayed.
  • If you return to work with restrictions, you must report those restrictions to Voya to obtain the appropriate documentation. The accommodation form must be completed within 15 days of receipt and returned to Voya at [email protected] or by fax at 855-882-8047 for processing.
  • If you exhaust both your Personal Leave and Sick Pay banks, coworkers may use the PL Donation Form to donate hours. The form is located here: Employee Authorization for PL Donation. Completed forms must be submitted to your regional Human Resources office for processing.
  • If you exhaust both your Personal Leave and Sick Pay banks, you will need to contact the Benefits Department to arrange payment for your benefits.

 

Leadership Communication from Voya Absence Management

Voya Leave Management communicates with Broward Health leadership regarding employees who are on leave or who have submitted leave requests. Leaders receive notifications and have access to reporting tools to help track employee absences and support workforce planning.

  • Leader Notifications

    Leaders receive email notifications when an employee initiates a leave request, when a claim decision is made, and when key updates occur during an approved leave. These notifications support awareness of employee availability and return-to-work planning.

  • Access to Leave Information

    Authorized leaders can view leave-related information through Voya’s secure reporting tools, including leave status, return-to-work timelines, and intermittent leave usage, based on their assigned access.

  • Reporting and Planning Tools

    Voya provides reporting capabilities that allow leaders to review current and upcoming absences, track intermittent leave usage, and support staffing and operational planning.

Frequently Asked Questions

How long does it take for an employee's FMLA request to be approved?

Once a leave request is submitted, Voya Leave Management will send the employee a claim packet and request any required medical certification. Per federal guidelines, employees have 15 calendar days to return the completed medical certification from their health care provider.

After sufficient documentation is received, Voya reviews the information and typically makes a determination within three business days. Approval timelines may vary depending on the circumstances of the leave and how quickly required documentation is provided.
