If you expect to be absent due to surgery, a medical procedure, or for 24 consecutive hours or more for family or personal medical leave, or if you are requesting a workplace accommodation, you must contact Voya Leave Management as soon as possible.
Before starting your request, review the Request for Leave Checklist to understand required notifications, timelines, documentation, and return-to-work steps.
You can initiate the leave process through:

For unplanned leave, contact Voya immediately. For planned leave, notify your department leader at least 30 days in advance and contact Voya to initiate your leave.

Once a leave request is submitted, Voya Leave Management will send the employee a claim packet and request any required medical certification. Per federal guidelines, employees have 15 calendar days to return the completed medical certification from their health care provider.
After sufficient documentation is received, Voya reviews the information and typically makes a determination within three business days. Approval timelines may vary depending on the circumstances of the leave and how quickly required documentation is provided.
For questions or assistance, employees may contact Voya Leave Management at (833) 973-1670 or the Broward Health Leave Administration Center at (954) 473-7701.
If an employee does not qualify for FMLA, Voya Leave Management will review the request to determine whether the employee may be eligible for another type of leave, such as Broward Health General Medical Leave or another applicable leave or accommodation.
If additional documentation or steps are required, Voya will notify the employee and coordinate with the Broward Health Leave Administration Center as needed. Eligibility and next steps will be communicated once the review is complete.
As a leader, you must not accept or retain any medical information from an employee regarding their leave. Employees should be directed to submit all medical documentation directly to Voya Leave Management or the Broward Health Leave Administration Center.
It is critical to maintain the confidentiality of employees on leave and not share any details with others in your department. If colleagues ask about the status of an employee who is out, you may acknowledge their concern but should explain that, as a leader, you are unable to share any information regarding the employee’s leave.
Leaders should refer to the Broward Health Attendance and Punctuality policy located in PolicyStat (PolicyStat ID: 17701727) and consult with their Chief Human Resources Manager for guidance.
If you are undergoing a one-day procedure, please complete the One Day Procedure form and return it to the Leave Administration Center at [email protected] for review and processing.
