Medical Staff Credentialing and Privileging

With the purpose statement of “We Ensure Exceptional Healthcare Providers,” our team of credentialing professionals in the Credentials and Qualifications Department (CQD) is here to assist with your Broward Health credentialing needs and questions. The CQD functions as the centralized verification office for processing credentialing and privileging applications in support of the regional hospitals within Broward Health and Broward HealthPoint. We do this with a focus on cost-effectiveness, efficiency, high quality, and customer service excellence.

Our general phone number is 954-473-7575 and general fax number is 954-473-7255. Our general email address is [email protected]

Reappointments and Miscellaneous Credentialing Requests

Existing medical staff members and Advance Practice Providers (APP) will receive an email notification about reappointment from the Credentials and Qualifications Department (CQD) approximately six months prior to expiration of their current appointment and privileges expiration date. If you do not receive this email notification, please contact the Credentials Specialist assigned to your file (based on your primary facility). It is important for you to return the completed reappointment application no later than the date indicated in the notification to allow sufficient processing and review.

Existing staff members and APPs should promptly contact their assigned Credentials Specialist to update their records whenever they have demographic or practice information changes, APP/physician sponsorship needs to be updated, or when a required reporting event occurs.

Refer to the Medical Staff Bylaws and Medical Staff Rules and Regulations if you have questions related to medical staff credentialing and clinical privileges.
