The CQD functions as the centralized verification office for processing credentialing and privileging applications in support of the regional hospitals within Broward Health and Broward HealthPoint. Meet the staff of the Medical Staff Credentials and Qualifications Department of Broward Health
Applicants desiring to practice at one or more Broward Health’s hospitals will start the process by requesting a pre-appointment application. This process is to review your qualifications and determine if you qualify to join the medical staff and/or have clinical privileges. The following disciplines may apply:
Physicians
Allopathic and Osteopathic Doctors, Podiatrists, Dentists and Oral-Maxillofacial Surgeons, Psychiatrists, and Psychologists.
Advanced Practice Providers
Physician Assistants and Advanced Practice Registered Nurses, including but not limited to Nurse Midwives, Certified Registered Nurse Anesthetists and Certified Anesthesiology Assistants.
For New Applicants and Refresher for Current Staff and APPs
Our Medical Staff and Advanced Practice Providers Applicants Orientation is designed to guide you through the application process and help you integrate seamlessly into our team.
Existing medical staff members and Advance Practice Providers (APP) will receive an email notification about reappointment from the Credentials and Qualifications Department (CQD) approximately six months prior to expiration of their current appointment and privileges expiration date. If you do not receive this email notification, please contact the Credentials Specialist assigned to your file (based on your primary facility). It is important for you to return the completed reappointment application no later than the date indicated in the notification to allow sufficient processing and review.
Existing staff members and APPs should promptly contact their assigned Credentials Specialist to update their records whenever they have demographic or practice information changes, APP/physician sponsorship needs to be updated, or when a required reporting event occurs.
