What to Know Before Your Visit
School physicals, sports physicals and Florida Certificate of Immunization are available for $50 each. Insurance does not cover these services, and payment is due at the time of the visit.
Please bring:
- The complete school or sports physical form for the provider to review and sign
- Your child’s most recent immunization record
- Your child’s prescription eyeglasses or contact lenses, if applicable
Location: Broward Health Urgent Care Weston, 2300 N. Commerce Pkwy., Weston, FL 33326
Hours: Monday through Friday, 8 a.m. to 8 p.m.; Saturday and Sunday, 8 a.m. to 4:30 p.m.
