The Community Care Plan (formerly known as the South Florida Community Care Network) is a collaborative effort between Broward County's two safety net hospital systems, Broward Health and Memorial Healthcare, serving the Medicaid population of Broward County.

What is The Community Care Plan

The Community Care Program (CCP) was the first Provider Service Network in Florida to serve Medicaid enrollees. It provides high standards of health care and health education and does what it can to promote healthy communities. The Medicaid managed care model helps people get the right care, at the right time, and in the right place, focusing on your satisfaction and health. The Community Care Plan is the ONLY Medicaid Plan that is owned and operated by local safety hospitals, making sure that everyone receives the care they need, no matter what their resources.
