Community Care Plan (CCP), known as the health plan with a heart, was established in 2000 and owned by two South Florida health care leaders, Broward Health (North Broward Hospital District) and Memorial Healthcare System (South Broward Hospital District). CCP is one of Florida's first and longest-standing Provider Service Networks and holds NCQA accreditation for service and clinical quality.
CCP serves 100,000+ members through Medicaid in 19 Florida counties, Florida Healthy Kids in eight counties and self-insured employer plans for Memorial Healthcare System and CCP employees. It also helps Broward County residents shopping for Affordable Care Act coverage and supports uninsured and uncompensated populations.
Its provider network includes participating physicians from Broward Health, plus additional benefits and community resources that help members get and stay healthy.
