View your health summary, review test results, and access trusted health information. With the new Clipboard feature, you can also update health issues, immunizations, allergies, and procedures before your scheduled appointment (if assigned by your provider).

Setting Up a New Account
If you are an existing patient of Broward Health and would like to request access to your medical records, please call the Health Information Management Department at the facility where you received treatment. You will also need to submit the following forms to receive access. Or, you may self-enroll from your computer or mobile device by entering the required information.
1. Patient Portal Consent Form
This form allows you or your representative access to the Patient Portal to view your health information.
2. Personal Representative Designation Form
The Personal Representative Designation form must be completed for all persons 18 and over who are NOT the legal Power of Attorney (POA) and who are NOT the patient.
Revoke Access to Patient Portal
If you no longer wish to participate in the Florida HIE, or have access to the Patient Portal, please fill out the Revocation of Authorization Form and submit it to your Health Information Management Department.
Make Changes to Your Medical Record
If you feel there are changes that should be made to your medical record, submit a Request to Amend Medical Information Form to your Health Information Management Department.

The patient portal offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With the patient portal, you can use the Internet to:

The patient portal is a free service offered to our patients.
You must sign a consent form upon admission and provide us with identification and a valid email address. You can also present in person with your identification at your participating Broward Health, Health Information Management (HIM) Department at any time (contact information on next page). Once registered, users will receive a custom invitation with instructions on how to complete the sign-up process. Or, you may self-enroll from your computer or mobile device by entering the required information.

Yes, you can. This is called proxy access and allows a parent, guardian or designee to log in to his or her own personal patient portal account, and then connect to information regarding the family member. Proxy access can be used to view health records for minor children and adult patients. Complete the appropriate Proxy/Authorization form(s) during the admission process. If you are requesting access after discharge, complete the Patient Portal Consent and Florida Health Information Exchange form(s) located on the patient portal and submit in person with your identification to your participating Broward Health, Health Information Management (HIM) Department. Or, you may self-enroll from your computer or mobile device by entering the required information.
The patient portal does not allow for a teen, aged 13-17, to view patient health information due to Florida Statue requirements regarding teen confidentiality. On your teen's 13th birthday, the Parent Proxy view will automatically be terminated.
No, due to the sensitive nature of medical information, each adult must sign and submit a Patient Portal consent form and establish their own account.
You may click the “Forgot password” link on the sign-in page to reset your password online. You may also contact Cerner’s Consumer Care (877) 621-8014 to request a new, secure password.
You may call your participating Broward Health, Health Information Management (HIM) Department to obtain a new invitation to the patient portal. If you have changed your email address recently, you must present in person and fill out another consent form with your new email address. Or, you may self-enroll from your computer or mobile device by entering the required information.
Log into the patient portal and on the top right corner, go to the Account Settings and select the appropriate option. Note: Updates to this setting do not update information on your medical record.
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal ID’s, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, the patient portal uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with the patient portal.
The patient portal is owned and operated by Broward Health partnered with Cerner corporation and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by the patient portal.
We aim to protect your privacy and security of your information. While logged into the patient portal, if your keyboard remains idle for 20 minutes or more, you will be automatically logged out of the patient portal. We recommend that you log out of your patient portal session and close your internet session if you need to leave your computer for even a short period of time.
You need access to a computer or mobile device connected to the Internet and an up-to-date browser (such as Microsoft Internet Explorer 11 and higher, Google Chrome, Mozilla Firefox, Apple Safari, or Apple Mac Mobile Safari).
For your security, your invitation expires after 14 days and is no longer valid after the first time you use it to create an account. If you still have problems, call your participating Broward Health, Health Information Management (HIM) Department (contact information on page 2). They will be able to tell you if you still have a valid Access Code or if you need to obtain a new one. Or, you may self-enroll from your computer or mobile device by entering the required information.

Florida Health Information Exchange (HIE)
Health Information Exchange (HIE) Consent - allows Broward Health to view your most complete and up-to-date medical information available from other providers participating in Florida State HIE.
Any decision that patients make to not grant authorization to Broward Health does not affect patient treatment or sharing of health information among your providers through other methods.
