Public Records Request

A request for public record can be emailed to [email protected] or by calling (954) 473-7303 directly.

In addition, you may submit your request via this form, online.
Your request will be logged and processed expeditiously. Please complete the information below: Your full name, email address, phone number, and specific record request. Provide as much information as possible about the specific record you are requesting.

The use of this online form is optional. The records custodian can be contacted at the above telephone number and/or email address.

Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing. See 688.6076, Florida Statutes.
